The COVID-19 Vaccine Program Act of 2021, also known as Republic Act 11525, requires the Department of Health to issue and verify certificates to everyone who has completed their vaccine.

VaxCert PH works with the Department of Information and Communications Technology (DICT) and the Department of Health (DOH) to create a digital VaxCert PH verifier that meets to World Health Organization requirements.

You can use VaxCertPH to show your individual status with full COVID-19 vaccinations. This is particularly important when traveling overseas, especially when paired with the International Certification of Vaccination (ICV), which acts as a vaccine passport for both international and domestic travel.

How can I get a VaxCertPH?

You can apply online or contact your local government for help. Because VaxCerts can be used as travel passes, you should carry one with you along with your passport when traveling.

Follow the steps below to register your VaxCertPH website:

  1. Visit https://vaxcert.doh.gov.ph/.
  2.  After agreeing to the privacy terms and conditions, you will be asked if you have been vaccinated for more than 48 hours.
  3. Select "yes" if it applies, and then input your personal information.
  4. Next, insert your vaccination information from your COVID-19 vaccination card. This includes the city or municipality where you were vaccinated, as well as the dates of your first and second doses.
  5. Once your vaccination facts have been reviewed and cross-checked with government records, you will be requested to submit a copy of your valid ID, vaccination card, and contact information.
  6. The certificate's validity is confirmed 14 days before your registration. You will then be able to get a QR code for your VaxCertPH.

How can I verify my VaxCert?

You will need a smartphone, tablet, or any device with a camera to complete your VaxCertPH. Here's how to validate your VaxCert!

  1. Open your mobile browser and go to https://vaxcert.doh.gov.ph/.
  2. Once you're in, you'll be asked if you want to request a vaccination certificate or confirm an existing one. Choose the second option.
  3. Give your phone's camera permission and press the scan button.
  4. After successfully scanning the QR code on your vaccination certificate, the application will display your information along with your QR code.
  5. Congratulations! Your verification request has been approved.
